One of the very first things patients ask me is: 'How long will I have to wear braces?' It's completely understandable — before you commit to treatment, you want to know what you're committing to. I always give an honest answer at the consultation once I've assessed the case, because every mouth is different. But here's a realistic overview of what to expect.

Average Treatment Times by Type

  • Metal braces: 12–24 months for most cases
  • Ceramic braces: 12–24 months (similar to metal)
  • Invisible aligners: 6–18 months for mild to moderate cases
  • Lingual braces (placed behind the teeth): 18–30 months
  • Early interceptive treatment in children: 6–18 months for Phase 1
  • Retainers (after active treatment): worn for life, but nighttime-only after the first few months

Durations are averages. Actual treatment time depends on case complexity and patient compliance.

These are averages. I've had patients finish in 10 months and others who needed 28 months. The key variables are the severity of the starting position and how well the patient follows the treatment plan.

What Determines Treatment Length?

Complexity of the Case

The biggest factor. Simple crowding of two or three teeth is very different from a full bite correction involving jaw discrepancies, rotations, and spacing across multiple teeth. During your consultation, I assess how far teeth need to move, whether bite correction is required, and whether any teeth need to be aligned in three dimensions — all of which affect the timeline.

Patient Age

Teenagers generally move teeth faster than adults. This is because bone density is lower during growth, allowing teeth to shift more readily. Adults can and do complete treatment successfully, but the biology is slightly slower. This doesn't mean adults should avoid treatment — it just means expectations need to be set accordingly.

Patient Compliance

For fixed braces (metal or ceramic), compliance mainly means attending your 4–6 weekly appointments and avoiding foods that damage brackets. For invisible aligners, compliance is far more demanding: trays must be worn for 20–22 hours every single day. Patients who wear their aligners for only 14–16 hours daily will find their treatment extends significantly.

Things That Can Delay Your Treatment

  • Missing or repeatedly rescheduling adjustment appointments
  • Broken brackets or wires that aren't repaired promptly
  • Not wearing aligner trays for the required hours
  • Poor oral hygiene leading to cavities that need treatment before orthodontics can continue
  • Eating hard or sticky foods that repeatedly damage braces
  • Not wearing elastics (rubber bands) as instructed — these are important for bite correction

The Retention Phase — Often Overlooked

Most patients focus on the active treatment phase — the braces or aligners. But equally important is what comes after. Once your braces come off, teeth will naturally try to drift back towards their original positions. This is called relapse, and it happens to everyone if retainers aren't worn. I fit every patient with custom retainers on the day their braces are removed and give clear instructions on how to wear them. Retainers worn consistently will preserve your result for life. Retainers ignored will undo years of treatment.

How to Stay on Schedule

  • Book your next appointment before you leave the clinic each time
  • For aligners, set a daily phone reminder to track wearing hours
  • Contact the clinic immediately if a bracket breaks or a wire pokes — don't wait for your next appointment
  • Follow diet advice — it's there for a reason
  • Wear your elastics every day if prescribed
